Forms & Insurance
At Diamond Bar Surgery Center our goal is to provide patients with the leading dental technologies while using the finest available materials in a soothing comfortable environment. To make the process as smooth as possible we ask that our patients, or their caregivers, be prepared to pay their estimated portion of the bill for treatment at the time of their appointment. Then, for your convenience, we will file the claim and bill your insurance company. We will always do everything possible to make sure the claim goes through, but in the event the insurance company does not pay it becomes the responsibility of the patient.
We understand that most insurance policies are provided through your employer so it is important to carefully read the terms of the contract, and we will help you to get the most out of your policy. We are also registered with all branches of the California regional center system for the developmentally disabled and will also accept Medi-Cal.
We have provided several important forms below that you should print out and read prior to your visit to our office as well as instructions following a routine wisdom tooth extraction.
